New Patient Information
Please arrive 5-10 minutes early for your appointment so you have plenty of time to fill out your intake forms.
If you have been injured in an accident, please let us know when you make your appointment.
If you have had x-rays taken recently (within the past 1-2 years) please bring them with you to your first appointment. You can check them out from the facility where they were taken.
Please plan at least 1 1/2 hours for your first visit.
Payment and Insurance
We accept the following insurance:
- Aetna
- Premera
- Lifewise
- Uniform
- First Choice
Bring your insurance card and a photo ID if you plan to use your health insurance coverage.
If you have another type of insurance, we can check if you have out of network benefits in our office.
Accepted forms of payment are cash, check, and credit card.
